Initial Assessment

The Client Assessment Process (about 1.5 hours)

Our Client Assessment Specialists bring some lightness and laughter to the initial assessment visit because we understand that it can be stressful to bring someone new into your home. We believe our assessment is the first step in creating a long term relationship of trust and caring.

Questions We’ll Ask:

  1. Personal background, hobbies & interests.
  2. What kind of help you would like?
  3. Current and past health information.
  4. Family contacts and physician information.

We’ll also answer all of your questions about our home care services and take a walk through the home.

Our goal is to gain a holistic understanding of the senior during the initial assessment process. The information gathered is vital to our mission of making a good match between the caregiver and senior. Together we’ll create a care plan that supports your needs with a caring, personal touch.

Service Agreement and Deposit

If you decide that you would like to start service with With A Little Help, our Client Assessment Specialist will go over our Service Agreement with you. We will also ask for a deposit equal to an estimated half month’s service or a minimum of $175. This deposit will be kept for the duration of service and applied towards the final bill.

Start of Service

We generally start providing care within a few days of the initial assessment, but sooner is possible if needed. One of our care coordinators will call to tell you about the caregiver who will be coming so that the transition to starting care is as simple as possible.
